About this course

Overview and Outcomes.

Overview

The shift from hospital to home succeeds or fails at the boundaries between organisations. This module sets out five steps of a good discharge, uses Senge's shifting the burden archetype to show how quick fixes weaken real solutions, and explores what matters over time and task through Cartrefi Cymru's home care review and Buurtzorg. It then turns to the hidden ninety per cent, Local Area Coordination and housing as part of health. When a partnership fails, families, regulators and auditors will ask who was responsible, and leaders need a clear, honest answer. This module maps who answers for what in England and Wales, sets out why a joint board does not merge its members' duties, and uses contribution rather than attribution to describe shared outcomes. It covers the ICO Data Sharing Code, joined-up data and five assurance questions for board members.

What you'll develop

In this Bitesize, you will develop the ability to:

  1. Set out the five steps of a good hospital discharge and explain why discharge is a shared regional responsibility rather than a hospital problem.
  2. Use the shifting the burden archetype to recognise how quick fixes in discharge and home care can crowd out fundamental solutions, and how to protect the slower work.
  3. Assess community-based and asset-based approaches, from the hidden ninety per cent and Local Area Coordination to Care and Repair's housing work, with honest attention to their evidence.
  4. Explain who answers for planning, money, social care, pooled funds, regulation, audit and voice in England and Wales, and why a joint board does not merge its members' duties.
  5. Distinguish when attribution is fair from when a contribution story is needed, and judge where a partnership sits on shared information and shared decisions.
  6. Apply the expectations of the ICO Data Sharing Code and the test of sharing as if the person were in the room, and use five assurance questions as a board member.
