About this course

Overview and Outcomes.

Who It's For

Leaders, board members and managers in the NHS, councils, social care, housing and the third sector in Wales and England.

Overview

This Focus brings together the practice of integration at the boundaries where care succeeds or fails. It opens with care closer to home: five steps of a good discharge, Senge's shifting the burden archetype, Buurtzorg, Local Area Coordination and housing as part of health. It then sets out who answers for what in partnerships, contribution rather than attribution and the ICO Data Sharing Code. It turns to leading through relationship rather than structure, with Huxham and Vangen and the Greater Manchester evaluation, and closes with Torbay's Mrs Smith, the Regional Integration Fund and Gwent as the first Marmot region in Wales.

Aims

This Focus aims to develop leaders who protect the slower work of prevention, give honest accounts of shared accountability and lead through trust rather than structure alone. It builds the habit of following one person's journey to find what a partnership has neglected and what to change.

In summary, you will be able to:

  1. Recognise where quick fixes in discharge and home care crowd out lasting solutions.
  2. Explain shared accountability in partnerships with clarity and honesty.
  3. Lead change from one person's journey across organisational boundaries.

What you'll develop

Through this Focus course, you will build the ability to:

  1. Set out the five steps of a good hospital discharge and explain why discharge is a shared regional responsibility rather than a hospital problem.
  2. Use the shifting the burden archetype to recognise how quick fixes in discharge and home care can crowd out fundamental solutions, and how to protect the slower work.
  3. Explain who answers for planning, money, social care, pooled funds, regulation, audit and voice in England and Wales, and why a joint board does not merge its members' duties.
  4. Explain why relationships are the infrastructure of integration, using Huxham and Vangen's distinction between collaborative advantage and collaborative inertia.
  5. Judge what a time-limited fund needs to leave a lasting legacy, and apply the design for Mrs Smith approach to a service or decision of your own.
  6. Use the six-move loop of leading integration to find what your partnership has neglected, and plan one change from following a single person's journey.
